Community Membership Consent and Waiver Form

Welcome to the Joytara Community! I am grateful you took this step of engaging with others and building your community. I look forward to getting to know you and supporting you on your journey.  Please look over this waiver form and feel free to ask any questions prior to signing. 

I agree to join the Joytara Community led by Bindiya Mahabir, Ed.M., BCBA, of Joytara Behavioral and Social Coaching.  I acknowledge this community offers participants with opportunities to make new friends and enhance their social interaction skills through structured activities but can not guarantee you will make friends. The monthly membership fee of $25 provides participants access to free and fee-based events, such as hikes, movie and game nights, restaurant and cafe meetups, rock climbing and cooking classes, bowling and much more! I understand that I have to RSVP for each event at least 1-week prior to each event date. I understand that some events have a maximum number of participants and therefore, I may not be able to attend all events. A social coach will be present at all official Joytara Community events to offer support as needed. 

Assumption of Risk

I understand and acknowledge that participation in the Joytara Community involves certain risks, including but not limited to emotional distress, social anxiety, or physical injury. I acknowledge that the social coach will take reasonable steps to ensure the safety and well-being of all participants.

Code of Conduct

In the service of creating a safe and inclusive group, I agree to be respectful, act maturely, maintain the confidentiality of other members, and refrain from taking pictures of others without their permission. If a member engages in any disrespectful comments or lewd actions, their membership will be terminated immediately and no refund will be given for that event or month. If you have any concerns, please notify Bindiya Mahabir immediately. 

Membership Cancellation Policy

Your membership will automatically renew at the end of each billing cycle unless you provide notice of cancellation in accordance with this policy. To cancel your monthly membership, you must email joytaracoaching@gmail.com. The cancellation will be effective at the end of the current billing cycle, and you will not be charged for any subsequent billing periods.

Refunds

Membership fees are non-refundable. If you cancel before the end of your current billing period, you will have access to the membership benefits until the end of that period but will not receive a refund for the unused portion of the membership. Joytara Behavioral and Social Coaching can not offer any refunds due to no-shows or cancellations for fee-based events that depend on the third parties' policies. If unable to attend, please contact Bindiya Mahabir at 925-315-7345 at your earliest convenience.

Release and Consent

By signing up, I agree to release and discharge Joytara Behavioral and Social Coaching and any of its employees/contractors, owner Bindiya Mahabir or the assistants, from any injuries sustained by me as a result of participating in Joytara Community events or activities. I have read and understood the information above and I agree that I am between the ages of 18-30 years old.
